The use of LC-MS/MS for quantitative analysis of small molecules in the diagnostic arena has taken off almost vertically in the last few years. Several large commercial diagnostic laboratories are now routinely running millions of samples a year by this tried and tested technology. However, one new frontier for LC-MS/MS is the analysis of proteins and peptides in the clinical laboratory. Unlike small molecules there is not a well understood pathway that can be borrowed from the pharmaceutical industry. Rather, new instrumentation and new approaches need to be utilized or often developed from scratch. To this end we will show some examples of the intricacies of developing, validating and deploying clinical protein/peptide LC-MS/MS assays and what we have learned from the journey.
