53. Analysis of Cortisol, Cortisone and Dexamethasone in Serum Using LC-MS/MS and Assessment of Cortisol: Cortisone Ratios in Patients with Impaired Kidney Function

A sensitive and selective LC-MS/MS method was developed to simultaneously quantify glucocorticoids cortisol, cortisone and dexamethasone in human serum. The limit of quantitation was 0.3 µg/L for cortisol and cortisone and 0.5 µg/L for dexamethasone. Total imprecision was less than 10.9%. Using this method, cortisol to cortisone ratios (CCR) in intensive care unit (ICU) patient samples was found to be 2 times higher than CCRs from out-patient samples. Statistically significant differences in concentrations of both cortisol and cortisone were observed in samples of ICU patients between groups with Glomerular Filtration Rate <15 and >90mL/min/1.73m2.
