69. Observations on the Metabolism of Morphine to Hydromorphone in Pain Patient Urinary Excretion Data using LC-MS/MS

Liquid chromatography tandem mass spectrometry (LC-MS/MS) is an accurate diagnostic tool used in urine drug screening of pain patients to determine adherence. In this study, a database comprising urinary excretion data was analyzed to determine the relationship between morphine and its metabolite, hydromorphone. Linear regression analysis of the data showed morphine metabolism to hydromorphone is a concentration dependent, saturable pathway. Mathematical modeling of extensive and poor metabolizers yielded an estimation of the percentage of extensive and poor metabolizers in the pain patient population. These findings provide insight into the prevalence of pharmacogenetic variability in the pain patient population.
