Fentanyl is a fast acting synthetic opioid widely used in medical care for anesthesia and analgesia, in cancer therapy and chronic pain management. To better understand fentanyl metabolism and for more accurate quantification of fentanyl and its metabolites in human urine, we used an Exactive mass spectrometer with ultra-high mass resolution (100,000) and sub-3-ppm mass accuracy coupled with high performance liquid chromatography. This is a simple dilute-and-shoot strategy and appears to be very sensitive in the detection of fentanyl and its known metabolites with the ability to identify unknown metabolites using simultaneously generated fragment ions.
