Short Abstract

Determination of drugs in the blood for the therapeutic treatment of epilepsy has an important diagnostic role . We developed quantitative method for detecting antiepileptic drugs by HPLC / MS. Analytical equipment - liquid chromatograph and a mass spectrometer LCMS-8060 (Shimadzu, Japan). Sample preparation - a protein precipitating from plasma/serum. This method is designed for the monitoring of the antiepileptic drugs phenobarbital, phenytoin, carbamazepine, lamotrigine in serum/plasma. The limits of detection are in the range of from 100-500 ng/ml depending on the analyte. The recovery rate was 93 - 98%. The CV of each quality control sample concentration does not exceed 5%. Our method are validated and suitable for routine analysis.

Long Abstract

Introdution. Therapeutic drug monitoring - a way to control and monitor the distribution of introduced into the body of drugs by determining their concentration in the blood. TDM is very important, if the drug has a narrow range of efficiency, a narrow therapeutic / toxic index. This analysis allows you to optimize the dosing regime, explain the lack of efficacy, prevent / explain the toxic effects, evaluate the impact of drug-drug interactions.

Methods. We developed quantitative method for detecting drugs by high performance liquid chromatography tandem mass-spectrometry (HPLC / MS). We are using analytical equipment - liquid chromatograph and a mass spectrometer with a triple quadrupole LCMS-8060 company Shimadzu (Japan). For selective detection is used MRM mode (Multiplied Reaction Monitoring). Fast sample preparation: only one step - protein precipitating from plasma or serum.

Results. This method is designed for the specific monitoring of the antiepileptic drugs phenobarbital, phenytoin, carbamazepine, lamotrigine in serum or plasma. The limits of detection are in the range of from 100 ng to 500 ng depending on the analyte. The recovery rate ranges from 93 to 98%. The coefficient of variation of each quality control sample concentration does not exceed 5%.

Conclusions. Therapeutic drug monitoring (TDM) provides valuable information on the actual blood concentration of a antiepileptic drugs. Determination of drugs in the blood for the therapeutic treatment of epilepsy has an important diagnostic role . With our LC/MS/MS method for determination of phenobarbital, phenytoin, carbamazepine and lamotrigine, we are getting reliable results in therapeutic drug monitoring efficiently. Our method are validated and suitable for routine analysis.
